We are in network with Delta Dental, Dental Care Plus/ Sunlife, Metlife, Humana, United Concordia, Superior, Some Anthem/Blue Cross Blue Shield plans. (Additional plans may follow, an in-network fee schedule.)
Yes, we will submit to out-of-net network insurances. We cannot guarantee what percentage of cleanings/ treatments they will cover. We can always request pretreatment estimates for any proposed treatment before scheduling. You will be responsible for any remaining balance.
You would have to contact Anthem and give them Dr. Jody’s name, address and phone number. They will tell you if we are in network. Out of Network plans could be self-pay if Anthem pays you, the subscriber.

Yes! We are currently accepting new patients. Please fill out an appointment request or call our office. We do not accept new patients over the age of 12.
If you are 10 minutes late for cleaning, we may only have time for X -Rays and an exam. If you are 15 minutes past your cleaning time, or 20 minutes late for a Treatment appointment, we would require you to reschedule. Please plan to arrive on time to avoid cancellations or fees.

If you do not cancel 24 hours prior during office hours, you could be charged a fee depending on the type of appointment. Our fee for Cleaning/Exam cancellations is $45.00 per child.

For treatment appointment cancellations fee is $75.00 per child.

Please keep in mind, when you cancel with such late notice, you are taking away an appointment from another child. We do understand that life happens. Your account will be reviewed before being charged.

Yes! We do request that a parent/legal guardian call in the day before or morning of the appointment to complete any health history updates as well as any consent forms needed prior to the appointment. We do require a parent/Legal Guardian for all New Patient appointments.

We will send you our new patient packet in advance to be completed. It will usually be texted/emailed 2 weeks before the appointment. If you would prefer to fill out a physical copy, we would request you come 15-20 minutes prior to the appointment to complete.

Reminder, we do require a parent/legal guardian for the 1st appointment.

We are located at the back side of the Ledgestone office complex at Suite 50. We can see the Coffman YMCA from our front door.
